Switzerland has been providing health-related assistance to Mozambique for 18 years and, since 1990, has gradually moved towards a budgetary and sector-based support focusing on primary healthcare, pharmaceutical needs and management. Switzerland has joined with other countries to establish common procedures, for example concerning the import of drugs and medical equipment. Better coordination between financial backers, public institutions and non-governmental organizations will result in more targeted and more effective action.

Since 1998, at the request of MISAU (the Ministry of Health) and various backers, and in order to give MISAU immediate access to the funds provided by these backers, Switzerland has provisionally assumed responsibility for managing the funds supplied by other external sources (Denmark, Ireland and Norway) for the purpose of importing drugs and medical equipment and financing current expenditure.

In recent years, Switzerland has provided average annual funding of CHF 11 million, of which CHF 9 million has been earmarked for the institutional component, primarily in terms of budgetary support. This is managed by the Ministry of Health (MISAU) and the Ministry of Planning and Finance (MPF). In parallel with this institutional assistance, the DCB likewise aims to foster community involvement so that greater consideration is given to the needs of the population.

Gaps still exist in the management and development of healthcare policies. Moreover, public-sector salaries are very low, which prompts trained healthcare workers to migrate to the private sector. But such considerations should not detract from the significant progress that has already been made: the donors’ activities are better integrated into the Ministry of Health’s programme, making for more effective healthcare services.

This fact sheet presents SMART (SolidarMed Antiretroviral Treatment) programme. The goal of this program is to strengthen the local health systems of nine rural districts in Tanzania, Mozambique, Lesotho and Zimbabwe, as well as to integrate voluntary counselling and testing, antiretroviral treatment, activities to prevent HIV transmission from the mother to the child, and home based care into routine health service provision. Solidarmed builds on established partnerships and cooperation in these districts with local hospitals and health administrations.

In Africa most importantly it appears that there must be a high political and moral commitment in order to sustain positively the impact of preventive measure against HIV/AIDS. Access to drugs (09.2004)
Many of the issues surrounding the accessibility of medicines in low income countries can only be addressed with concerted national and international action. In this paper we consider the issues of accessibility and availability of pharmaceuticals in international health, and describe the initiatives that have been taken by SDC to address them in Tanzania and Mozambique.
